Supporting people to feel safe, build independence, and stay connected to their routines, their community, and what matters to them.
In this role, you will support participants in a SIL and community-based environment across Inner City Melbourne, working with individuals with complex support needs. You will play a key role in creating structure, building confidence, and supporting participants to navigate both their home and the community with consistency, dignity and care.
You’ll work closely within a team to deliver person-centred support that is respectful, reliable, and tailored to each individual’s goals and needs.
Responsibilities include: - Providing consistent, person-centred support in line with individual care plans and behaviour support strategies
- Responding to behaviours of concern in a calm, proactive and respectful manner
- Encouraging engagement in meaningful activities at home and in the community
- Assisting with daily living tasks such as meal preparation, prompting personal care and maintaining a safe home environment
- Maintaining accurate documentation, including progress notes and incident reporting
- Working collaboratively with the broader team to ensure consistency in approach and quality of support
About You:To be successful in this role, you will have:
- A valid P2 or Open driver’s licence and reliable vehicle
- NDIS Worker Screening Check and Working with Children Check
- Experience supporting individuals with complex needs in a disability, community or similar setting
- Strong communication, patience and rapport-building skills
- Availability for a variety of shifts including mornings, afternoons and evenings (including active overnight support)
