Service Coordinator

Kanda is seeking an experienced Service Coordinator to deliver high-quality in-home care, with a strong focus on rostering and workforce coordination. As part of the Veterans and Aged Community Care Team, this role manages rosters, supports service delivery, and enables clients to live safely and independently at home

  Key Responsibilities
  • Lead and support field staff to deliver safe, client-focused non-clinical care, working effectively within a team to ensure consistent service outcomes
  • Coordinate rosters, shift allocations, and cancellations in an efficient and timely manner
  • Maintain accurate client records and ensure daily visits are completed and properly documented, promptly escalating any handover concerns to the Clinical Team Leader
  • Develop and maintain positive relationships with clients and their families
  • Provide guidance and mentorship to staff, promoting professional and safe work practices
  • Maintain accurate records to meet legislative, NDIS, and audit requirements
  About You
  • Experience in community, aged, or disability care coordination
  • Demonstrated capability in organisation, time management, and communication
  • Compassionate and client-focused, with strong capability to guide and mentor staff
  • Current valid Driver’s Licence, NDIS Worker Screening Check, and Working with Children Check
