Support Coordinator

Access Support Coordination is seeking a passionate Support Coordinator to join our team in Bundaberg, QLD. In this full-time role, you will guide participants in implementing their NDIS plans, ensuring they understand their options to make informed choices. You will coordinate the implementation of these supports, optimising capacity and funding management.

Key Responsibilities
  • Help participants build meaningful, connected lives beyond funded supports
  • Guide participants in implementing their NDIS plans, ensuring they understand their options and can make informed decisions
  • Coordinate supports effectively, with a focus on maximising capacity and managing funding efficiently
  • Assist participants in gathering relevant evidence to reflect changing needs and support future funding requests
  • Identify and facilitate opportunities in areas such as recreation, employment, and housing to enhance quality of life
  • Advocate for participants to ensure mainstream services meet their responsibilities and commitments
About You
  • Demonstrated experience in a Support Coordination role or similar within the Disability sector
  • Qualification in Disability, Social Work or Community Services (or related)
  • Strong understanding of the NDIS framework and its processes
  • Confident in meeting KPIs including billable hours (highly regarded)
  • Excellent communication skills with a genuine commitment to finding solutions for our clients
  • Hold a valid drivers licence, Working with Children Check and NDIS Worker Screening
