All Plan Management is seeking an experienced Support Coordinator to step into a Partnerships Lead role — a relationship-focused position where you’ll represent the organisation across the sector, strengthen referral pathways, and champion exceptional customer experience.
About the roleAs Partnerships Lead, you’ll be the key contact for providers, referrers, and sector partners. You’ll enhance trusted relationships, support sustainable growth, and work closely with Marketing, Onboarding, and Service Delivery teams to ensure referrals convert smoothly and customers feel supported.

Key Responsibilities - Build and manage strong referrer and stakeholder relationships as a trusted point of contact
- Grow referral pathways through strategic partnerships and industry engagement
- Collaborate with internal teams to ensure seamless referral conversion and service delivery
- Improve retention and experience by proactively addressing needs and reducing churn
- Capture insights to drive improvements, support reporting, and strengthen team capability
About You - Demonstrated experience within Support Coordination or Plan Management with a strong understanding of the NDIS framework
- Proven ability to build trusted relationships and represent the organisation across the sector
- Creative & innovative with experience driving customer experience initiatives that improve retention
- Strong collaboration skills with internal departments & experience using CRM systems and managing stakeholder pipelines
- Willingness to travel within Queensland and interstate as required
