TCP Coordinator

Kanda is seeking a compassionate and organised Registered Nurse to join our Veterans', Aged Care and community nursing team as the TCP Coordinator. This full-time role coordinates the Transition Care Program ensuring clients receive safe, timely, and personal- centred support when transitioning from hospital to home.

Key Responsibilities

Coordinate TCP client assessments and care plans
Liaise with hospital teams, discharge planners, and allied health professionals
Arrange services with internal teams and external providers
Monitor client progress, review care plans, and adjust supports as needed
Maintain accurate documentation, complete reporting, and manage invoicing
Identify and report risks or incidents, promoting safe practices

About You

Current AHPRA registration as RN (no conditions)
Strong knowledge of aged care, My Aged Care portals, referral pathways, and community supports
Excellent communication, stakeholder management, and organisational skills
Current Driver’s Licence with reliable vehicle
Hold or willing to obtain: National Police Clearance & Working with Children Check
