TCP Coordinator

Kanda is seeking a compassionate and organised Registered Nurse to join our Veterans', Aged Care and community nursing team as the TCP Coordinator. This full-time role coordinates the Transition Care Program ensuring clients receive safe, timely, and personal- centred support when transitioning from hospital to home.

Key Responsibilities
  • Coordinate TCP client assessments and care plans
  • Liaise with hospital teams, discharge planners, and allied health professionals
  • Arrange services with internal teams and external providers
  • Monitor client progress, review care plans, and adjust supports as needed
  • Maintain accurate documentation, complete reporting, and manage invoicing
  • Identify and report risks or incidents, promoting safe practices

About You
  • Current AHPRA registration as RN (no conditions)
  • Strong knowledge of aged care, My Aged Care portals, referral pathways, and community supports
  • Excellent communication, stakeholder management, and organisational skills
  • Current Driver’s Licence with reliable vehicle
  • Hold or willing to obtain: National Police Clearance & Working with Children Check
