Your role
  • Client Assessment and Rehabilitation: Evaluate client needs and design personalised exercise and rehab programs for daily activities such as dressing, mobility, and meal preparation
  • Home Safety and Equipment: Conduct home safety checks, prescribe equipment, and train staff and carers on its use
  • Care Planning and Review: Keep client care plans current and aligned with best practice
  • Collaboration: Work closely with our Home Services team, TCP Coordinator, and multidisciplinary partners including medical practitioners and hospital discharge planners
  • Training and Support: Mentor and support RNs, ENs, Care Workers, clients, and carers
  • Documentation and Administration: Complete client records, manage equipment, and coordinate brokered services
  
What we are looking for
  • Current AHPRA registration as a Physiotherapist with no conditions
  • Bachelor of Applied Science in Physiotherapy or equivalent
  • Minimum two years’ experience as a Physiotherapist
  • Excellent communication and customer service skills
  • Accountable, reliable, and committed to delivering high-quality care
  • Enthusiasm for ongoing professional development
