Support Worker

We are currently seeking Support Workers to join our team across Melbourne’s Western suburbs, including Fraser Rise and Melton South, supporting clients with a range of disability support needs in both home and community settings.

About the Role
In this role, you’ll provide individualised, person-centred support to participants in their homes and the community. You’ll work as part of a supportive team, delivering consistent care while maintaining professionalism, strong boundaries, and safety at all times.

Key responsibilities include:
  • Providing daily living and personal care support as required
  • Supporting routines, independence and overall wellbeing
  • Assisting with community access, appointments and social inclusion
  • Implementing care plans using trauma-informed and person-centred approaches
  • Collaborating with team members and documenting support provided accurately

Requirements
  • Valid driver’s licence and reliable vehicle
  • NDIS Worker Screening Check and Working With Children Check (or willingness to obtain)
  • Ability to work across a flexible roster, including day, afternoon and night shifts
  • Proven experience in disability support, including SIL, residential, community, or complex care settings
  • Experience supporting individuals with complex needs, high physical support, behaviours of concern, or psychosocial disabilities
