We are currently seeking compassionate Support Workers to join our team in
Ivanhoe East, supporting a client with
complex cognitive and
physical support needs in a home-based setting.
About the Role In this role, you’ll provide individualised, person-centred support in a community-based home setting. You’ll work as part of a supportive team, delivering consistent care while maintaining professionalism and strong boundaries.
Key responsibilities may include: - Providing daily living support, including personal care and self-care prompting
- Supporting routines, safety and overall wellbeing
- Assisting with appointments and community-based activities as required
- Following individual support plans using person-centred approaches
- Working collaboratively with the care team and maintaining accurate documentation
Shifts Available
- Morning shifts: 6:00am – 2:00pm
- Afternoon shifts: 2:00pm – 10:00pm
- Sleepover shifts included (exit at 6:00am)
- Ongoing weekly roster with consistent hours
- Coverage required Monday to Sunday

Requirements: - Valid driver’s licence and reliable vehicle
- NDIS Worker Screening Check and Working with Children Check (or willing to obtain prior to commencement)
- Availability to work a rotating roster including morning, afternoon and sleepover shifts
- Previous experience in disability, aged care, community or related support roles
- Confidence supporting individuals with higher support needs in a professional, person-centred manner
