About the Role Kanda is seeking an enthusiastic
Home Care Packages (HCP) Coordinator to join our Veterans', Aged Care, and Community Nursing Services team. This full-time role is crucial for planning and delivering direct care services for HCP recipients, ensuring high levels of safety, compliance with Aged Care Quality Standards, and continuous improvement. You'll work closely with the Senior Clinical Nurse to manage scheduling and recruitment needs, continuously grow our HCP client base, and provide high-quality, person-centred customer service.
Key Responsibilities - Client Plan Management: Guide participants in implementing their NDIS (HCP equivalent) plans, helping them understand options, and optimising capacity and funding.
- Service Coordination: Coordinate the implementation of supports, supporting participants to collect evidence for changing needs and funding requests.
- Customer Engagement: Develop strong relationships with clients and families, ensuring their needs are met with urgency and quality outcomes. Manage all client onboarding and exit tasks.
- Compliance & Quality: Ensure all client files, assessments, and service agreements are fully compliant and accurately maintained in line with Aged Care Quality Standards.
- Workforce Support: Work with the Senior Clinical Nurse to manage incidents, client feedback, and service delivery for assigned employees, supporting regional workforce strategies.
- Administration & Reporting: Utilise systems to record interactions accurately, undertake administrative tasks, update client files/assessments, and conduct client-specific reporting.
About You - Relevant Health or Community Care qualification; or commensurate experience in a similar role.
- Proven experience in a similar role within the community, aged, or disability care environment.
- Strong communication skills that demonstrate a commitment to care and empathy for clients and staff.
- Outstanding organisational and time management skills.
- Exceptional attention to detail.
- Ability to respond to client needs with a sense of urgency and provide quality outcomes.
- A proactive approach to continuously grow the client base.
- Hold or willing to obtain a NDIS Worker Screening Clearance and an unrestricted Driver’s License.
